OFFICE POLICIES
The following policies have been implemented to streamline the function of the office thereby allowing staff members to focus on the needs of patients rather than administrative work. Please help us to better care for you by following these policies.
CO-PAYS
When you arrive at the office, please take care of your co-pay by means of cash or personal check. The office is not able to accept debit or credit card payments. If you cannot take care of your co-pay at the time of your appointment, please mail it to the office as soon as possible. Given the costs associated with billing for co-pays, a five dollar service charge will be added to accounts billed for co-pay collection.
HAVE YOU MET YOUR DEDUCTIBLE?
On some insurance policies, charges for office visits are applied to a policy holder's deductible. If you have such a policy, until your annual deductible has been met you will be responsible for the charges for your office visit. If you have not met your deductible, please make an effort to take care of your office charges at checkout.
LATE CANCELLATIONS
Cancellation or rescheduling of appointments within 24 hours of the appointment of the scheduled time keep other patients from having access to needed appointments. They commonly occur as a result of a patient forgetting to get lab work done. Please be courteous and plan ahead. While it generates extra work for the nurse, it is possible for us to obtain lab results done as late as the day before the appointment. Unless a legitimate reason is provided for a last minute cancellation, a twenty five dollar charge will be added to the patient's account.
